Role OverviewThe Associate Care Coordinator assists low risk Enrollees, appropriate for care coordination and care management services, in achieving their optimal level of health. The Associate Care Coordinator must have relevant experience and education to work telephonically and face to face with Enrollees to be able to complete surveys/assessments and update a plan of care for low risk members.
ResponsibilitiesBachelor's Degree (or higher) in a health or social services related field. Certification as a Case Manager or a Community Health Worker preferred.
LicensureValid driver's license with car insurance.
Experience1 to 3 years social services and/or care management/coordination experience working with clients in a home/community setting, including those receiving physical health, behavioral health and social services preferred. 3 to 5 years of Case Management experience preferred.
