Are you the kind of person who is able to work with individuals of varying backgrounds to work towards a common goal for the betterment of a client?As a WCBH Care Coordinator, you'll work closely with our Clinical Team to provide care coordination and case management services for a caseload of clients receiving community based and in office skills-based therapy. The primary role of the Care Coordinator will be to assess needs, coordinate care and connect individuals with community resources. The Care Coordinator will also support clinical processes associated with mental health treatment when applicable. Essential Duties and Responsibilities:Help clients and families understand treatment options and make informed decisions through regular in office and phone check ins.Work with treatment team to facilitate coordination of care and information sharingParticipate in the development and implementation of treatment plans, case management, and other clinical documentationPromote effective utilization of clinical resources, community resources and other social supports.Assist clients with resources by sharing information and applying for benefits, when necessary.Care coordination/communication both internally and externally to facilitate continuity of services.New Competitive CompensationRequirementsEducation:Associate's degree in Social Work, Rehabilitation, Psychology, Education, Criminal Justice, or Human Services related field with 2 years work with person(s) who have severe mental health disability or understanding of mental illness acquired through provision of support to family members with mental illness.Bachelor's degree in Psychology, Social Work, Education, Rehabilitation, Human Services or related field is required with 2 years of experience preferredExperience:2 years' related experienceRequired Skills:Excellent communication, problem-solving, time management, and organizational skills required.Must be flexible and demonstrate the ability to work with minimal supervision.Excellent computer skills and knowledge of Microsoft OfficeAbility to learn necessary computer and technical skills essential to the position and Agency needs.Ability to relate to staff, clients and community around a mental health system.Maintain skill competencies in customer service, quality improvement, compliance and safety.Preferred Skills:Knowledge of and experience with electronic medical record system, preferred.Experience providing targeted case management services.Prior experience in a community mental health environment.Knowledge of evidence-based mental health treatments such as Dialectic Behavioral Therapy (DBT), Cognitive Behavioral Therapy (CBT), Enhanced Illness Management and Recovery, and Prolonged Exposure for PTSD.Physical Requirements:Ability to access all Agency sitesOther:Ability to travel in connection with the job duties assignedHave and maintain a valid state driver's licenseHave reliable transportationHave and maintain personal automobile insuranceAbility to obtain and maintain Agency and/or State of New Hampshire required background check(s)Understanding of the importance of confidentiality and the ability to maintain that confidence.
