Become the Care Coordination Specialist You Aspire to Be

  • Collaborate with an interdisciplinary team to create and adjust treatment plans based on patient needs and preferences.
  • Establish and refine tentative discharge plans in coordination with your team.
  • Participate in the planning and execution of a seamless patient discharge experience.
  • Monitor patient experiences regarding quality, timeliness, service appropriateness, payor expectations, and more.
  • Facilitate weekly team conferences and coordinate treatment plan modifications.
  • Complete necessary documentation and case management addendums.
  • Stay updated on regulations, standards, and company policies.
  • Analyze case management reports and implement appropriate actions.
  • Understand commercial contract levels, patient exclusions, payor requirements, and recertification processes.
  • Engage in Acute Care Transfer meetings to identify trends and solutions.
  • Meet with patients and families shortly after admission to assess needs.
  • Conduct goal assessments and documentation within established timelines.
  • Educate patients and families about rehabilitation and the Care Coordination Specialist's role while creating communication plans.
  • Schedule family conferences when necessary for comprehensive care.
  • Assist patients in timely resource acquisition to avoid discharge delays.
  • Ensure compliance with regulations regarding orthotics and prosthetics.
  • Facilitate timely referrals, including documentation to post-discharge providers.
  • Ensure accuracy in discharge and payor-related information in patient records.
  • Participate in the utilization review process, focusing on data collection and trend analysis.
  • Contribute to the case management on-call schedule as necessary.

Qualifications

  • License or Certification:
  • Must be proficient to conduct independent assessments according to your professional discipline (e.g., RN, SW, OT, PT, ST, or Rehabilitation Counseling).
  • If required by your profession's state regulations, you must possess an active license.
  • Must be eligible for CCM® or ACM certification at the time of entering this role or within two years of taking the position.
  • CCM® or ACM certification is required or must be obtained within two years of moving into the Case Manager II role.

Minimum Qualifications:

    • Nursing roles require a minimum of an Associate Degree in Nursing; BSN preferred, as well as licensure. A diploma is acceptable only in states where this is the minimum requirement.
    • For other eligible licensed or certified healthcare professionals, a minimum of a bachelor's degree is required, with a graduate degree preferred.
    • Preferred experience includes at least 2 years in rehabilitation settings.
