Position Summary:Necco has an opportunity for a career as a Bridges Case Manager. This role will be working with transitional age youth. Ridiculously passionate about the company's success, growth, and program quality.The candidate selected will be responsible for the following:Intake and EnrollmentFacilitate enrollment and eligibility in timely manner for young adults referred to the program.Coordinate housing and case management needs with young adults.Assist in preparation of court related materials and appear in court with the young adult for any Bridges related hearings.Assist teammates with their job duties whenever possible.Service DeliveryMaintain files in accordance with all Bridges compliance standards.Respond to and document all critical incidents in accordance with Bridges and Necco protocols.Encourage and support young adults participation in focus groups, independent living classes, and other support groups.Ensure Bridges eligibility documents are entered and maintained in the SACWIS system in a timely manner.Complete tier identification, VPA, case planning, and other assessment tools and updates.Ensure face to face visits meet or exceed Bridges compliance standards.Ensure cases are closed when eligibility standards are not met or upon the young adult's request.Professional DevelopmentParticipate in professional development activities and complete required trainings.Attend regional information sessions with collaborative members and ODJFS.Build rapport with Bridges participant and Bridges Regional Coordinator.Establish community resources.Team ResponsibilitiesAttend and participate in self-directed daily huddles/weekly tactical/monthly strategic meetings and activitiesPerform other duties and responsibilities as assigned by immediate supervisorParticipate in achieving our mission: We Build FamiliesParticipate in the performance quality improvement process and execute any assigned tasks related to the processEnsure that all employee-related documents are current and up to date for your employee personnel fileCorporate CitizenPractice ruthless pragmatismEngage in peer to peer feedbackKnow and live the Necco Corporate Culture PrinciplesEmbody the three essential virtues of humble, hungry and smartDrive your Individual Performance ScorecardAdhere to and contribute to the Necco meeting structurePosition Qualifications21 years of ageBachelor's Degree in a human services-related field and a minimum of 2 years experience working with the older teenage populationValid Driver's LicenseLimited Liability Auto Insurance Coverage of 100/300/100Organizational, and Written/Oral Communications skillsMust have the skills necessary to navigate and execute several software's (i.e. Microsoft offices, Databases, Electronic Health Records, Dashboards, etc.)Successful completion of all required Criminal Background checks and drug screenings per state and company regulationsAbility to adhere to scheduled and unplanned deadlinesWillingness to travel and attend trainingSuccessful completion of all required training pertaining to jobAt Necco, we value diversity and are committed to creating an inclusive and equitable work environment.
