Deliver comfort and dignity - one patient at a time LPN - Hospice - PRN - Flexible Schedule - Dayshift We're looking for a Hospice LPN to join our team. You'll report directly to the Administrator or Executive Director and play a key role in providing compassionate, hands-on care to patients and their families. This position supports end-of-life care in the setting patients call home - whether that's a private residence, assisted living, or facility. As a Hospice LPN, You Will:Provide direct patient care services to hospice patients and supportive care to families with a reasonable caseload. Assist the interdisciplinary team in evaluating patient/family needs and contribute to the plan of care. Report findings or changes in the patient's condition to the Registered Nurse. Deliver care with empathy and attentiveness in accordance with the patient's individualized care plan.
