MINIMUM QUALIFICATIONS:

• Bachelor's degree in Human Services or a related field preferred.
• At least one year of experience in community involvement and/or working with clients, or an equivalent combination of education and experience.
• Bilingual in English and Spanish required.
• Valid Colorado Driver's License at time of appointment and throughout employment.
• Knowledge of care coordination and community resources.
• Proficiency in MS Excel, MS Word, MS Outlook, and general database reporting systems.
• Strong communication skills, both oral and written, with the ability to engage with clients and community partners effectively.
• Ability to maintain confidentiality and handle sensitive information in accordance with Colorado State and Federal laws.
• Ability to manage multiple tasks, work independently, and collaborate as part of a team.
• Ability to perform physical tasks such as lifting, bending, walking, and driving as required by the job.

EDUCATION, EXPERIENCE, AND FORMAL TRAINING:

• Bachelor's degree in Human Services or related field preferred and/or at least one year experience in community involvement and/or working with clients or an equivalent combination of education and experience.
• Bilingual (English/Spanish)
• Care Coordination

LICENSES OR CERTIFICATES:

• Valid Colorado Driver's License at the time of appointment and throughout employment.

POSSIBLE WORK SCHEDULE: Flexible work schedule with regular hours being on weekdays and occasional weekends and evenings.
