This is an exciting opportunity for a motivated individual with a passion for community outreach, aging in place, and housing rehabilitation to make a meaningful difference in the lives of older adults. This position provides critical support to Civic Works' housing programs (Elder Services Program). The ideal candidate will be self-motivated, solution-oriented, and have prior experience working with seniors or in a human services setting. This role includes both field and office work, with regular travel throughout Baltimore City and partnership sites. Personal transportation is required (mileage reimbursed provided).   • Manage a caseload of older adult clients and provide application assistance for home repair and support services
  • Conduct community outreach to identify eligible clients and build program awareness
  • Develop relationships with community groups, agencies, and service providers
  • Participate in events, health/resource fairs, and outreach activities
  • Assist clients with navigating benefits and accessing additional support resources
  • Advocate on behalf of clients and ensure timely follow-up
  • Accurately maintain documentation and case notes
  • Support the planning and execution of outreach events and marketing
  • Travel regularly throughout Baltimore City to meet clients and attend partner meetings
QualificationsMinimum Requirements
  • High school diploma or GED required; college coursework or degree preferred
  • At least 1 year of case management or related human services experience
  • Experience working with older adults
  • Passion for supporting seniors and promoting dignity and independence
  • Strong interpersonal and communication skills
  • Dependable, flexible, and self-motivated
  • Valid driver's license and access to reliable transportation
  • Able to pass a drug test and criminal background check
  • Experience with database software or client management systems
  • Proficiency in Microsoft Office (Word, Excel)
