Job Summary The Behavioral Health Navigator will play a vital role in supporting individuals receiving outpatient mental health services by helping them access care, coordinate services, and overcome barriers to treatment. The Navigator serves as a liaison between clients, clinical providers, and community resources to ensure a seamless continuum of care and to promote engagement, retention, and positive outcomes in behavioral health treatment.
Qualifications Required:
- Bachelor's degree in Social Work, Psychology, Public Health, or related field
- 1-2 years of experience in behavioral health, case management, or care coordination
- Strong interpersonal and communication skills
- Knowledge of mental health, substance use disorders, and community resources
- Ability to work independently and collaboratively in a fast-paced environment
- Proficiency in documentation and use of electronic health records
Preferred:
- Bilingual
- Experience working with underserved or high-risk populations
- Familiarity with trauma-informed care, motivational interviewing, or harm reduction principles
