As a Behavioral Health Consultant (BHC), you'll be an integral part of our medical primary care team, providing mental health support through the Primary Care Behavioral Health (PCBH) model. This role centers on fast paced, real time consultation and brief mental health interventions that meet patients where they are. In addition, you'll also provide short term therapy when appropriate, especially for patients identified through warm handoffs or follow-ups. Therapy is considered a secondary function, with primary focus on real time access and support for our medical providers.Essential FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.Delivers integrated clinical services within the PCBH model at our Eastside location, focus on real time access to care, same day consultation and brief interventions.Collaborate closely with medical providers, MAs, and nursing staff to address mental health, substance use, health behavior change, life stressors, and stress related physicalIdentify patients who would benefit from behavioral interventions, offering individual, couples, and group sessions tailored to their needs.Identify patients appropriate for brief behavioral health interventions and short term therapy, offering services individually, or in couples and group formats when appropriate.Conduct behavioral health screenings, brief assessments, and referrals to specialty services asConsult with primary care staff and coordinate care to support holistic health andProvide support for patients in crisis, including real time safety planning and connection to emergency services when needed.Serve as a reliable and flexible member of the care team, available for real time response during clinic hours (no on call hours).Maintain a small short term caseload, as appropriate to site needs and aligned with the BHCAdapt to various responsibilities in a fast paced, evolving workUtilize your professional credentials (LPC, LPCC, LGSW, LICSW, LMFT) to focus on effective case management and care coordination.Support team efforts related to partnerships, outreach, marketing, and program growth, helping expand the impact of behavioral health services within the community and across theKey CompetenciesKnowledge of principles and best practices of integrated behavioral health in a community-based setting using the Primary Care Behavioral Health Model (PCBH).
