Treatment planning, discharge planning and referrals: - Develop and maintain the patient's treatment plan throughout the patient's course of treatment with input from the care team - Keep patient, family, or others informed as to the patient's progress throughout treatment as the patient desires. - Complete safety planning - Complete discharge planning - May lead psychoeducational groups - Coordinate patient's social determinants of care needs - Practice de-escalation techniques - Provide patient information and make referrals - Understand Medicaid and other governmental benefit systems and help patients navigate through the public mental health system. - Help patients follow through with their health goals. - Coordinate medical and behavioral healthcare for patients - Assists patients with activities of daily living - Conducts assessments including but not limited to suicide screening and mental health assessments, patient rights, obtaining releases of information, advanced directives...Mental Health, Mental, Patient, Behavioral Health, Behavioral, Counselor
