Overview:

The Team Lead of Health Equity & Associate Program Director of Health Equity, for the Department of Medicine, being informed/guided by the OBH SVP, Health Equity, Population and Community Health,is responsible for developing and leading health equity initiatives across the Dept. of Medicine and Ambulatory Services and is also responsible for the well-being, education, and professional development of Interfaith internalmedicine residents, with a particular focus on teaching and promoting the health equity of our community. The above comprises 50% of the effort associate with administration & teaching. The other 50% is comprised of patient care activities at OBHs various clinics or inpatient campuses.

Responsibilities:

Professional Services

  • Serving as a Medicine primary care physician at one or more of OBHs various clinics or inpatient campuses.
  • Supervising residents and medical students.
  • Performing and participating in quality programs of the Department and Hospital.
  • Developing referral relationships with OBH colleagues and community providers.
  • Demonstrating leadership in the delivery of care that complies with all quality initiatives, with special emphasis on the Quality Incentive Programs (QIP) with which we participate.
    • This includes developing an in-depth understanding of the EPIC requirements for optimal documentation and charge capture necessary to achieve QIP goals and incentives.
    • This includes optimizing patient length of stay, reducing readmission rates, and helping to ensure patients have appropriate continuity of care post discharge.
  • Timely completion and closure of patient charts is mandatory and frequent delinquencies are cause for formal disciplinary action.

Department of Medicine Team Lead, Health Equity

  • Developing and leading health equity initiatives across the Dept of Medicine and Ambulatory Services, including:
    • Assessing opportunities to improve OBHs telehealth/MyChart access and use, as well as other access barriers that disproportionately affect the most vulnerable members of our community.
    • Partnering with Ambulatory and Business/Clinical Transformation leaders to improve care delivery with a focus on our most vulnerable patients.
    • Supporting institutional diversity, equity, inclusion, and belonging efforts.
    • Identifying and removing race correction (algorithms) with OBH
    • Proactively reaching out to current patients adversely affect by these algorithms
    • Educating employed/voluntary clinicians, residents about race, racism, and effects on health outcomes
    • Developing ways that clinicians/residents can get our patients involved in their care and aware of the impacts of racism
  • Facilitating continuous improvement and the spread of best practices, standard work, and lean ambulatory flow, including:
    • Leading and supporting ambulatory process improvement initiatives.
    • Coaching and supporting front-line providers and team members in adopting standard work and moving into lean ambulatory flow.
    • Contributing to ongoing quality improvement efforts as requested by the Chairperson or IMC Clinical Director.
  • Timely completion of all reporting and compliance requirements, including but not limited to, physician time studies, medical record reviews, coding queries, compliance certifications, and timely completion and closure of patient charts.
  • The compliance of the Department with all applicable Federal, state, and local laws, ordinances, bylaws, rules and regulations, and policies and procedures established by all governmental agencies.
  • Supporting the establishment and maintenance of utilization review and medical audit processes for the Department, and the monitoring of all quality assurance activities in the Department.
  • The preparation of periodic reports concerning the operation of the Department, as well as periodic reports to the Hospitals Medical Board or other designated committee(s) regarding the operation of and quality of care provided within the Department.
  • Other services as reasonably requested by the Chairperson of Medicine or IMC Clinical Director.

Teaching Services

  • Teaching responsibilities shall include, but not be limited to:
    • Providing leadership to the Residency Program.
    • Developing, teaching, and administering the OBH health equity curriculum to include:
      • Developing formal health equity curricula for all OBH residents on their ambulatory long block.
      • Developing a dedicated health equity track for interested residents within the overall ambulatory long block structure.
        • Mentoring trainee research and QI equity focused projects.
        • Supporting resident education in health equity.
  • Providing clinical supervision of the residents in the Program.
  • Assisting the Hospital in recruiting a sufficient number of appropriately trained and qualified faculty members to ensure the highest standards of education and clinical quality.
  • Assisting the Hospital in maintaining accreditation from the Accreditation Council for Graduate Medical Education and its Residency Review Committee.
  • Updating the evaluation process for individual resident performance and participating in the regular evaluation and annual promotion of residents.
  • Regular clinical competency evaluations of the residents.
  • Representing the Program on Hospital committees.
  • Monitoring the effectiveness of the curriculum and incorporating the results of the program evaluation into appropriate updates in the curriculum.
  • Structuring, coordinating, and providing lectures and interactive educational sessions with residents and students in the Program.
  • Promotion of the Program by becoming involved in various community and professional activities as necessary to meet the needs of the Program.
  • Participating in research and scholarly activities to enhance the academic reputation of the Program.
  • Providing such other educational and teaching services as may be reasonably required by the Chairperson of Medicine or the Program Director.
  • All other duties as requested.
Qualifications:

Education:

  • MD or DO required.

Knowledge and Skills:

  • Strong technical knowledge of the area(s) being managed. May serve as the subject matter expert but typically relies on others under management for subject expertise.
  • Ability to establish goals, set priorities, oversee the short and long term deliverable of department objectives, ensure optimum performance of staff, may operate in a project manager role as needed, coach others, ensure quality of results and output.
  • Solid understanding of the operation of the assigned department.
  • Requisite expertise and documented educational and administrative experience acceptable to the Review Committee.
  • Ability to fully utilize OBHs electronic record systems, equipment and other healthcare and billing systems relevant to this position.

Licenses,Certifications:

  • Current active certification in the specialty of Internal Medicine by the American Board of Internal Medicine.
  • Current medical licensure in the State of New York and appropriate medical staff appointment
  • Board certification required

Physical Requirements:

  • Position requires prolonged periods of standing, reaching, walking throughout the working day.
  • Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
