General Summary

The Application Assistance Navigator is responsible for helping families complete applications for programs such as CHIP, Medicaid and other services. This position will also conduct outreach to educate individuals and families about CHIP, Medicaid, and other resources and encourage their participation.

Primary Responsibilities

  • Provide application assistance and facilitate enrollment of eligible children and their families in the CHIP/Medicaid program.
  • Responsible for submitting a designated number of applications on a monthly basis and meeting any other benchmarks as determined by the supervisor to support the achievement of the program goals and objectives.
  • Tracks and reports required data as determined by the supervisor to show progress towards program goals and objectives. The Application Assistance Navigator is responsible for updating all relevant information on a weekly basis using the designated data reporting tool.
  • Responsible for maintaining excellent record keeping and documentation.
  • Implement outreach strategies, including but not limited to attending public events to promote the availability of CACOSTs application assistance services and educate the target population.
  • Develop and maintain collaborative relationships with community partners to support outreach efforts including schools, clinics, and local nonprofit organizations.
  • Responsible for addressing any areas of improvement needed in program implementation as identified by the supervisors ongoing monitoring for continuous quality improvement.
  • Responsible for participating in training and professional development which includes onboarding, new hire training, and ongoing individualized professional development to maintain knowledge and expertise in eligibility, enrollment, and program specifications for CHIP, Medicaid, and any other type of state or federally funded program, as assigned.
  • Complete the initial and annual training and certifications required via the Community Partner Website (CPP) to be a Site Navigator within the required timeframe, as specified by the Community Services Coordinator.
  • Responsible for maintaining the confidentiality of client records and adhering to the agency confidentiality policy.

  • Work Experience

    Required:

    Previous experience assisting vulnerable individuals and families with community resources and services

    Preferred:

    Prior experience with CHIP/Medicaid enrollment or other state/federally funded programs

    Prior experience in community outreach/education

    Education/Certifications/Licensure

    Required:

    Associates degree or sufficient college hours.

    Must complete training and certification required to be a Site Navigator via the Community Partner Program website upon hire.

    Preferred:

    Bachelor's degree in social work or a related field.

    Excellent computer skills and knowledge of Microsoft and other software tools;

    Excellent communication and interpersonal skills;

    Ability to establish and maintain professional relationships with individuals of varying social and cultural backgrounds and with co-workers at all levels

    Ability to make presentations and/or educate groups of people on available services;

    Ability to maintain confidentiality;

    Ability to learn new tasks quickly and efficiently;

    Strong multitasking skills and ability to meet deadlines.

    Preferred:

    Bilingual in English/Spanish

    Physical Requirements If a preliminary offer of employment is extended, a physical will be required before the offer can be accepted.

    Must have a valid drivers license, reliable transportation, a safe driving record, and be eligible for insurance coverage in the operation of Agency vehicles.

    Must be able to pass a criminal background check and physical examination.

    Able to perform the following physical requirements of the position with or without reasonable accommodation: critical sensory requirements include general vision; specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Hear or listen in the normal range (corrected) and speak and give directions clearly. Able to walk, sit, stand, kneel, crouch, crawl, reach, lift, carry, push, pull or otherwise move objects up to 10 lbs., and occasionally lift and/or move up to 25 pounds.
