This position coordinates care for patients across the lifespan who would benefit from high-touch collaboration, advocacy, and care coordination. Special areas of focus are patients with high risk medical needs (eg multiple chronic conditions, cognitive difficulties, independence concerns, suicidality, etc.), and those with complex social determinants of health needs or barriers (e.g. abuse and neglect, homelessness, financial insecurity, etc.).
