Job Description :
Purpose: Support the Medical Specialties Clinic in delivering high-quality clinical care to patients with rheumatologic and autoimmune conditions. This role includes a combination of direct patient care and administrative responsibilities, with a focus on collaborative, evidence-based management of chronic and complex diseases. Responsibilities: Provide outpatient care to patients seeking diagnosis and treatment for rheumatologic conditions such as rheumatoid arthritis, lupus, vasculitis, and other autoimmune diseases. Work closely with rheumatologists and other specialists to ensure accurate diagnosis, effective treatment, and continuity of care. Collaborate with a multidisciplinary team?including physicians, pharmacists, social workers, and care coordinators?to develop and implement comprehensive, patient-centered treatment plans. Educate patients on disease processes, medication management (including biologics and immunosuppressants), and lifestyle modifications. Monitor disease activity and treatment response using clinical assessments, laboratory data, and patient-reported outcomes. Adjust treatment plans as needed in consultation with supervising physicians. Participate in quality improvement initiatives and contribute to the development of clinical protocols and patient education materials specific to rheumatologic care. Track patient progress and coordinate care across the continuum, including referrals to physical therapy, infusion services, or specialty care as needed. Support the education of medical students, residents, and fellows through clinical teaching, case discussions, and participation in academic activities. Stay current with advances in rheumatology through continuing education and professional development. Perform administrative tasks such as documentation in the electronic medical record (Athena preferred), coordination of prior authorizations, and participation in clinic operations meetings. Other related duties as assigned. Required Qualifications: Licensed and certified Nurse Practitioner or Physician Assistant in the state of Texas, in good standing. Graduate of an accredited nursing or physician assistant program; bachelor's degree required. Strong clinical judgment, critical thinking, and problem-solving skills. Ability to work independently in a fast-paced, team-oriented environment. Excellent communication, organizational, and time-management skills. Commitment to patient-centered care and continuous quality improvement. Preferred Qualifications: Three (3) years of experience in a rheumatology, internal medicine, or other relevant outpatient specialty clinic. Familiarity with value-based care models and multidisciplinary care coordination. Experience with Athena EMR and tracking patient-reported outcomes. Bilingual in English and Spanish. Experience with infusion therapy coordination and biologic medication management.
