Summary:
The Advanced Practice Provider (APP) functions as a member of an inter-professional team to provide comprehensive health assessment, diagnosis and treatment of individuals with acute, sub-acute and advanced illness. APP provides medical care in support of professional medical staff. The palliative care APP will ensure that all dimensions of patient and family care (physical, emotional, social, practical, and spiritual and information) are considered, to assess and address these needs in accordance with the wishes of patient and family/caregivers, in collaboration with other members of the multidisciplinary team. The APP will contribute to teaching as requested. APP additionally provides a variety of medical services to patients in accordance with established laws and procedures. Works cooperatively with other health professionals under the direct supervision of a physician. APP will, as directed, review EPIC reports to screen for patients who would be appropriate for palliative care consultation.
Responsibilities:
EDUCATION
Graduate of an accredited RN program, MSN in Nurse Practitioner required, specialty area of Adult Acute Care preferred
Current NP license in the State of New York required
BCLS validation

Graduate of an approved NCCPA (National Commission on Certification of Physician Assistants), CAHEA (Committee on Allied Health Education and Accreditation of the AMA)
BCLS validation
EXPERIENCE
Three (3) years experience as a Nurse Practitioner or PA preferred
Advanced content knowledge in palliative care and hospice
Certification in CPR and annual re-certification required
ACLS certification preferred

Knowledge of Home Care and Hospice services
Knowledge of community resources
WORKING CONDITIONS:
Frequent sitting, standing and walking
Visual and auditory acuity are essential
Works in office setting and patient care units throughout facility
Exposure to infectious diseases and uses appropriate personal protective equipment
Working hours are varied, works beyond normal working hours, weekend coverage as necessary
