JOB TITLE: Adult Health Homes Care Manager CLASSIFICATION: Full Time, Non-ExemptPROGRAM/DEPARTMENT: Health Home Care Management IMMEDIATE SUPERVISOR: Program ManagerPOSITION SUMMARY: Works with peoplewith qualifying Physical/Mental Health/Substance Abuse/Developmental Disability diagnoses and their natural supports to link/monitor/refer to services that meet their individual needs and personal goals. Also provides ongoing advocacy.QUALIFICATIONS:Minimum Education, Training & Experience:Associates' Degree in a Human Services related field.One to Two (1-2) years of relevant experience in Human services (including but not limited to Mental Health, Social Work, Substance Use, Education, etc.)., with a minimum of 6-12 months experience developing Care Plans/SMART Goals and functional knowledge of community resources.****For applicants with an Associate's degree, a waiver is required to be completed and accepted by The New York State Department of Health before hire.Preferred Education, Training & Experience:Bachelor's Degree of Arts or Science in a Human Services FieldTwo (2) years of relevant experience in Human services (including but not limited to Mental Health, Social Work, Substance Use, Education, etc.), with a minimum of 6-12 months experience developing Care Plans/SMART Goals and functional knowledge of community resources.****Applicants with a Bachelor's Degree with less than 2 years' experience may be considered, but a waiver is required to be completed and accepted by The New York State Department of Health before hireESSENTIAL SKILLS AND ABILITIES:Valid New York State driver's license and dependable transportation that meets the Agency's driving policyAbility to work flexible hours and travel throughout Cattaraugus, Allegany and Chautauqua CountiesAbility to work effectively and independently including address multiple tasks efficientlyPossesses strong organizational, analytical and creative thinking skills Ability to effectively communicate verbally and in writing including developing rapport with the people we serveAbility to represent self and agency in a professional manner Possesses strong knowledge of community resourcesWillingness and ability to participate in an on-call rotationSALARY:$21.84 per hour starting rate Full Job Description Available Upon Request Benefits - Intandem Contact us for more InformationAn equal opportunity employer.
