Woodfords Family Services is seeking a qualified Adult Case Manager to join our Community Case Management team! This is a full-time (40 hrs.), salaried position, and includes a generous benefits package!
This position is based out of our offices in Westbrook, ME. Travel in and around Cumberland County as necessary is required.
Case Managers at Woodfords serve as an essential resource for individuals living with intellectual and or developmental disabilities. Services rendered include identifying consumer needs, coordinating appropriate supports and resources, and navigating consumers and their families through various components of the individual person-centered-plans (PCP).
The Case Manager serves a key communicator and advocate for adults and their caregiver(s) by ensuring all areas of development are met and managed with the appropriate resources. This is achieved by working with adult consumers and the treatment team to identify individual goals that promote independence and sustainability.
