Admissions Coordinator POSITION SUMMARY The Admission Intake Coordinator (AIC) will receive referrals being made to facilities for SNF placement and provide support to the admission teams and Patient Transition Liaisons to ensure admissions are received and assessed in a timely manner. The AIC will run background checks, perform insurance eligibility verifications, and enter information into the electronic medical records system to prepare the referral for admission to the facility RESPONSIBILITIES

• Referral and Admission Support: Processes incoming referrals by verifying insurance, conducting background checks, obtaining prior authorizations, and entering information into IRM.
• Communication and Coordination: Provides timely updates to the Admissions Team and assists Patient Transition Liaisons in gathering documentation related to admissions and costs.
• Professional Conduct and Confidentiality: Maintains courteous, professional interactions with all stakeholders and ensures confidentiality of resident and personnel information.
• Facility Compliance: Keeps accurate departmental records for each resident, including admissions and financial documentation, and participates in audits and inspections.
• Team and Quality Involvement: Attends departmental and quality assurance meetings, identifies improvement areas, and contributes to policy development and compliance.
• Workplace Culture: Promotes a positive, inclusive work environment focused on strong customer service and team support.

QUALIFICATIONS

• High school diploma or equivalent.
• Proficient computer skills and excellent time management are required.
• Experience with health insurance, Medicare, Medical Assistance, and other health care plans is preferred
• Experience in healthcare, skilled nursing facility, or assisted living community
