Professional Nurse Practitioner / Board-Certified APRN Spencer, New York Are you an experienced and compassionate Nurse Practitioner (NP) eager to make a lasting impact in a close-knit community? Your Role Will Include: Providing Comprehensive Patient Care:
  • Perform thorough physical exams and health histories
  • Assess patient symptoms, diagnose conditions, and develop treatment plans
  • Order and interpret diagnostic tests (blood work, imaging, etc.)
  • Prescribe medications and therapies for both acute and chronic conditions
  • Provide disease management services and follow-up care
  • Offer preventive care such as immunizations, screenings, and wellness checks
Empowering Patients Through Education:
  • Teach patients and their families about their health conditions and treatment options
  • Encourage healthier lifestyles and habits for long-term health and wellness
  • Offer guidance on self-care, nutrition, and disease prevention
Collaboration with Healthcare Teams:
  • Work alongside physicians, specialists, and other healthcare professionals to coordinate care
  • Provide valuable input in team meetings and patient care discussions
  • Offer mentorship and guidance to other clinical staff when needed
Ongoing Monitoring & Adjustments to Care:
  • Regularly assess patient progress and modify treatment plans accordingly
  • Maintain detailed, accurate documentation in electronic health records (EHR)
  • Ensure compliance with healthcare regulations and confidentiality standards (HIPAA)
Qualifications:
  • Master's degree in nursing from an accredited Nurse Practitioner program
  • Active New York Nurse Practitioner license
  • National Board Certification (e.g., Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, etc.)
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications preferred
  • Excellent clinical skills with strong diagnostic and decision-making abilities
  • Ability to work independently while collaborating in a team environment
