*Healthcare Consultant III - A-Line Staffing Solutions : Job Details

Perform field case management with elderly and individuals with disabilities approved for in-home or nursing facility care, involving travel to members' homes. Assess members' needs and eligibility using care management tools, evaluate benefit plans, and recommend appropriate services and interventions. Identify high-risk factors and escalate quality of care issues through established channels; present cases at multidisciplinary conferences to optimize outcomes. Coordinate and monitor care plan activities, ensuring progress and addressing barriers through collaboration with health professionals. Use motivational interviewing and coaching techniques to promote member engagement and healthy lifestyle changes. Critical Information Location: Work From Home with travel to counties in Illinois, including Southside/ Westside of Chicago and DuPage County.
