Integrated Case Manager – Inpatient (RN or LMSW)

• Location: Detroit, MI
• Work Type: Onsite / Contract (10/14/2025 – 01/10/2026)

Experience

• Acute inpatient case management: Must have at least 5 years, with recent experience within the last 2 years.
• Experience in discharge planning and care transitions.
• Exposure to complex patient populations (multiple comorbidities, high-risk cases).
• Optional but valuable: home health, rehab, community health, or managed care experience.

Epic EMR or other hospital EMR experience.

Education

• Nursing degree (BSN preferred if converting to full-time later) OR
• Master's in Social Work (MSW) for LMSW candidates.
