IDR Healthcare is seeking a travel nurse RN Home Health for a travel nursing job in Fond Du Lac, Wisconsin.

Registered Nurse – Hospice / Home Health

Schedule: Monday–Friday, 8:00am–4:30pmShift Requirements: No on-call or weekend shiftsWorkload: Average of 5 patient visits per day

Responsibilities
  • Provide compassionate nursing care to patients in home, hospice, or community-based settings.

  • Conduct comprehensive assessments and develop individualized care plans.

  • Deliver wound care, pain management, and other skilled nursing interventions.

  • Collaborate with physicians, social workers, chaplains, and other members of the care team.

  • Educate patients, families, and facility staff on disease processes, symptom management, and end-of-life care.

  • Document all care activities accurately in the electronic medical record.

  • Coordinate care transitions to ensure appropriate levels of treatment and support.

  • Advocate for patients by protecting their privacy and ensuring compliance with regulatory standards.

Qualifications
  • Current, unrestricted Registered Nurse (RN) license (state-specific).

  • CPR/BLS certification required.

  • Valid driver's license, reliable vehicle, and active auto insurance.

  • Experience in wound care required.

  • Prior hospice, home health, or case management experience strongly preferred.

  • Familiarity with electronic medical records (EMR) systems; Home Care Home Base (HCHB) highly desired, but training available.

Preferred Skills
  • Experience caring for elderly and medically fragile patients in home or facility settings.

  • Strong documentation skills, including narrative assessments and interdisciplinary group (IDG) reporting.

  • Comfort with care planning and coordination across multiple disciplines.

  • Knowledge of end-of-life medications, symptom management, and patient/family education.

IDR Job ID #463491. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
