Job Description & Requirements**Hospital at Home is designed for patients who do not require hospital admission but still need ongoing monitoring. RNs oversee patient care remotely from the Command Center in Pennsauken and respond in person when necessary. This is not a home health position; candidates must have MS/(Tele experience. The role involves approximately 50% in-home patient visits and 50% remote monitoring from the Command Center. Candidates must reside within a 50-mile radius of the facility to ensure familiarity with the service area.**Will be onsite at one of our five locations functioning as a care coordinator when not in patients homeWill float to LRM (Longitudinal Monitoring) and TDC tele discharge coordinationRTO due at time of submissionRadius Requirement: Must reside within 50 miles of the facilityPosition is for 4 12's per week, Traveler MUST be willing to work a 48hr/wk contractHours rotating 7a-7p and 10a-10pRequirements:Must have MS/Tele experience2yrs min experienceBLSValid drivers licenseExperience with lines and drains, IV insertion, phlebotomyEPIC charting experienceEOW/EOHFirst time traveler accepted but not preferredScrub color navy blue
