Job Type: Full-Time Contract

Shift: Monday – Friday, 9:00 AM – 5:00 PM

Pay: $66+ per hour

Travel Package: Weekly stipends available for qualifying travel candidates

Referral Bonus: $500 for qualified referrals (100+ hours worked)

Overview:

A leading NYC hospital is seeking a Licensed Master Social Worker (LMSW) for a full-time contract role in an acute care setting. This position offers the opportunity to make a direct impact on patients' lives by providing advocacy, discharge planning, and coordination of community resources. Ideal candidates are compassionate, organized, and experienced in hospital-based social work.

LMSW Social Worker Key Responsibilities:

• Review admission information to identify patient needs, including dietary, cultural, and religious considerations
• Serve as a patient advocate and advise care teams on patients' psychosocial needs
• Conduct discharge counseling to ensure patients have appropriate support, supplies, and accommodations at home
• Connect patients with community resources, government agencies, and nonprofits addressing housing, food insecurity, unemployment, or transportation barriers
• Coordinate care across multiple providers for complex patient cases

LMSW Social Worker Qualifications:

• Master's Degree in Social Work – Required
• Current LMSW license – Required
• 1–2 years of prior experience as a hospital/acute care Social Worker – Required
