Pride Health is hiring Home Health RNs to support our client in Jackson, MI 49201. This is a 3+Months assignment (Possibility of extension or conversion). opportunity and a great way to start working with a top-tier healthcare organization!

Position Summary

The Home Health Registered Nurse (RN) provides skilled nursing care in the patient's home under the supervision of the Nursing Supervisor. The RN ensures comprehensive and coordinated care by assessing the need for additional allied health services such as physical therapy, occupational therapy, nutrition, speech therapy, and social work.

Key Responsibilities

• Deliver direct nursing care to patients in the home environment.
• Assess patient needs and coordinate services with interdisciplinary teams.
• Educate patients and families on disease management, care plans, and treatments.
• Provide IV therapy and monitor patients' responses.
• Complete and maintain Oasis documentation accurately and in a timely manner.
• Utilize Epic EMR for patient charting and care coordination.
• Collaborate with physicians, allied health professionals, and caregivers.
• Transport, move, and assist patients as needed.
• Ensure quality, safe, and compassionate care while maintaining compliance with organizational and state requirements.

Qualifications

Required:

• Current RN licensure in Michigan
• BLS certification (AHA)
• 2+ years of recent Home Health RN experience.
• Oasis, IV therapy, and Epic experience.
• Minimum of nine months of general clinical nursing experience.
• Reliable transportation, valid Michigan driver's license, and satisfactory driving record.
• Ability to lift, carry, and move patients when necessary.

Preferred:

• Acute care experience.

Education:

• Completion of an accredited RN program (National League of Nursing).

Schedule: 8 hrs/Day – Day shift

Expected hours: 40 hours in a week

Mileage Reimbursement: $0.67/mile

Local Pay range: $45-$48/hour

*Offered pay rate will be based on education, experience, and healthcare credentials.

Travel Gross Pay: $2000 - $2160/Week
