Through this community of healers, scientists, and scholars, Rutgers is equipped as never before to transform lives.Posting Summary Rutgers, the State University of New Jersey, is seeking a Patient Navigator for the Infectious Disease Department within New Jersey Medical School. The Patient Navigator provides clients with medical case management services as defined by the Health Resources and Services Administration (HRSA). Provides client-centered services that link clients with health care, psychosocial, and other services. Among the key duties of the position are the following:
  • Coordinates/facilitates and follows-up on medical treatments and patient referrals for core medical services and other services as needed.
  • Answers patient telephone calls. Correctly processes information for appointment scheduling, answer questions, and assist with problem solving. Screens emergent/urgent phone calls, inquiries related to the availability of appointments, scheduled appointment dates and times. If necessary transfers calls to an appropriate person or department to assist the patient.
  • Assists physicians with all secretarial/coordination aspects of patient care for patients and follows physician protocols according to diagnosis and referral.
  • Documents all aspects/encounters accurately in the electronic medical record within appropriate timeframes.
  • Maintains a high level of professional responsibility and performance.
  • Participates as an active member on committees assigned.

  • Bachelor's Degree in a Health, Public Health, Social Work or other relevant field required; plus two (2) years' experience in the development and/or operations of community programs required.
  • Equivalent education, experience, and/or training may be substituted for the degree requirements.

  • Computer literacy with competence in Microsoft Office components required.

Preferred Qualifications
  • Phlebotomy certification is a plus.
  • NJDHSS HIV counselor certification strongly preferred.
  • Knowledge of Newark community preferred.
  • Bilingual skills (English/Spanish) preferred.
Equipment UtilizedPhysical Demands and Work Environment PHYSICAL DEMANDS:
  • Computer literacy, typing, working at multiple workstations in a shared environment, ability to speak clearly, write proficiently.
  • Extensive reading and extended sitting, ability to move from unit to unit.
  • Position requires standing and walking over 2/3 of the time. Talking, hearing & sight are essential; requires clear close & distance vision, peripheral & color vision, depth perception and ability to adjust focus.
  • Position requires lifting to 10 lbs; on occasion more based upon patient demands.
  • Assist with transportation services in the clinic.
WORK ENVIRONMENT:
  • Occasional (less than 1/3 of time) exposure to bloodborne pathogens that requires use of personal protective equipment.
