Williamsburg, Virginia Patriot's Colony in Williamsburg, Virginia is seeking a full-time Social Worker. The social worker works under the supervision of the Director of Social Services or Administrator to assist each patient/resident in reaching their maximum level of social and emotional functioning by promoting, interpreting and advocating their needs to family members and LLHARS interdisciplinary team. It is also the role of the social worker to help promote a smooth working relationship between the Department of Social Services, physician and other disciplines within the facility. Those tasks include discharge planning, care planning, discharge notifications, PASRR forms, Medicaid applications, psychoactive monitoring, behavior monitoring, social and psychosocial needs of the residents. B. S. W. , B. A. in Social Work from a program accredited by the Council on Social Work Education or other related field preferred.
