272 bed LTC Post acute care7 units includes2 secure dementia unitsPost acute rehab, a lot of pts from hospital (2,3, 5, 7 mix short term rehab )Dementia, Chronic Illnesses, Diabetes, RehabPointClick EMR
RN supervisor (they are alone on off shift (nights, weekend, holidays) so they are tending to any acute change like a fall, need to go ER, Head to toe assessment, calling provider, getting them over to ED and LPN
