Job Title: Registered Nurse Profession: Nursing Specialty: Long-Term Care Duration: Full Time Shift: Days Hours per Shift: 8 Experience: Minimum of 1 year as an RN in a long-term care facility License: New York LPN Licensure Certifications: BLS (AHA or RQI only) Must-Have: LTC experience, EOW and holiday availability required Description: The RN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served. The RN must possess the ability to assess data reflective of the patient's status and interpret the information needed to identify each patient's requirements relative to age-specific needs. The RN will provide direct and indirect skilled nursing care for assigned residents. The RN follows defined policies and procedures under the supervision of a Registered Nurse. The RN maintains a collaborative relationship with other nursing staff, physicians, and ancillary staff. The RN must maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs. The RN shall have the ability to provide care to geriatric residents. Job Duties: The RN collects data relevant to the resident's physical and psychosocial condition on an ongoing basis. The RN communicates findings to the Supervisor and other members of the Interdisciplinary Team. The RN contributes to the creation of a developmentally appropriate and culturally relevant plan of resident care. The plan reflects nursing diagnosis and interventions to achieve desired resident outcomes. The RN involves the resident/family and healthcare team in this process while promoting a healthy, safe environment. The RN recommends revisions to the plan of care after evaluating resident outcomes. The RN indicates the need for referrals to ensure all aspects of the resident's care have been addressed. The RN collaborates with the resident/family and members of the Interdisciplinary Team within the scope of nursing practice. The RN assures customer satisfaction. The RN maintains current knowledge and competency in Nursing Practice. The RN participates in the mentoring/orientation of staff. The RN considers factors related to safety, cost, and impact in planning and delivery of care. The RN provides leadership in the professional practice setting under the direction of the Nurse Manager. The RN demonstrates knowledge and competency in practice. Occupational exposure to blood-borne pathogens is reasonably anticipated. Required tasks may involve contact with blood, body fluids, or tissues.
