Job Summary:

A facility in New York, NY is seeking an experienced and compassionate RN Adult Care Nurse to join their Home Care team specifically for the Queens Borough. This role is primarily focused on providing home health care to patients, requiring strong clinical skills and the ability to work independently in a field-based setting. The ideal candidate will have extensive experience in acute care and a passion for providing hands-on care in a home environment.

Key Responsibilities & Qualifications:

• Experience:
  • A minimum of two years of previous acute care experience is required.
  • Experience in one or more of the following areas is essential: Med/Surg, Maternal and Child Care, Critical Care, or Pediatrics.
  • Current Adult Care home health experience is also required.
  • The nurse must be able to manage an average of 4-8 cases per day.
• Skills:
  • Proven ability to work independently in the field.
  • Excellent communication and interpersonal skills to educate patients and caregivers on health management, disease prevention, and care plans.
• Licensure & Certifications:
  • A valid and current New York State RN license is required.
  • AHA BLS certification is required.
  • Primary source verification of the license from the NYS Department of Education (NYSED) is also required.
