Seeking MSN prepared, Board Certified, NY State licensed Nurse Practitioners (Internal Med., Adult Health, Community Health, Family Practice, and/or Psychiatry) with 2 years (3600 practice hours) or more. NP must have an active Medicaid ID Number.

Preferred experience includes working with adult/geriatric populations, home health, and individuals with physical or developmental disabilities, along with knowledge of long-term care services and programs. Extra consideration for Spanish, Russian, Haitian-Creole, Chinese, Korean, and/or Bengali speaking NPs.

Summary

Seeking availability of either 9 - 21 hours/week; operates 7 days/week (90-minute increments, minimum 3 hours/shift). Laptops provided.

This position supports the statewide Independent Practitioner Panel that issues physician orders for individuals who have undergone a Community Health Assessment and are eligible for community-based long-term care services and supports. The incumbent will review the completed Community Health Assessment tool in the NYS Uniform Assessment to assist the Physician in completing a Physicians order. Follow-up with individuals assessed by an RN will also be required.

Job Responsibilities

• Review and make recommendations on consumer assessments for long-term care services and supports.
• Maintain current knowledge of regulatory changes related to assessment quality and validity.
• Assess cases and utilize clinical competencies to review and validate assessment outcomes.
• Collaborate to monitor and address clinical quality and gaps in care.
• Provide clinical and educational support for project initiatives.
• Interpret age-specific responses demonstrating effective communication skills.
• Participate in performance improvement and quality assurance activities.
• Support program goals through clinical and educational efforts.
• Meet all established standards per performance criteria and bonus templates.
• Perform additional duties as assigned by management.

Flexible scheduling, all liability insurance covered, 1099 independent contractor role.

Earn up to $2000/week.
