NYIAP - RN Home Health, Nurse AssessorQueens - Long Island City
- Perform in-home, in-facility, or telehealth assessments that include evaluation of individual's health status, strengths, care needs, and preferences using the UAS-NY assessment tool within the prescribed timeframes of the assessments.
- Establish and maintain positive relationships with consumers, caregivers, and facility staff.
- Regularly review medical documentation, reporting any concerns, conflicting information, or issues that may arise during the evaluation process.
- Serve as the lead content knowledge expert concerning the review and research of provided clinical information.
- Perform clinical case reviews and consultations.
- Provide direct supervision to assigned team of nursing and/or other clinical staff.
- Provide subject matter expertise related to medical necessity and level of care criteria and application thereof.
- Provide education and clinical training to staff in area of expertise.
- Enter assessment data into the UAS-NY online application and transmit, as required.
- Code the UAS assessment correctly based on training and guidelines in the UAS-NY Community Health Assessment Reference Manual.
- Review non-qualified assessments with the Clinical Quality Assurance Department.
- Maintain a comprehensive working knowledge of community resources, payor requirements, and network services for target population.
- Maintain updated knowledge of the UAS-NY assessment process.
- Review consumer medical documentation and/or health referral forms, as relevant to the case.
- Document any concerns, conflicting information, other issues that surface during the evaluation process.
Requirements
- Two (2) years of experience in a HOME setting within the last ten (10) years
- Working in the field for a Certified Home Health Agency (CHHA), Licensed Home Care Service Agency (LHCSA), or Managed Long-Term Care (MLTC)
- Conducting assessments that are similar to the UAS such as OASIS
- Providing primary care to homebound persons
- Care management for persons receiving home and community-based services including Hospice and Assisted Living
- Home care experience as a paraprofessional (e.g., Home Health Aide) or Licensed Practical Nurse caring for a minimum of 2 patients per week
