POSITION AND DUTY REQUIREMENTS

  • Clinical Personnel Required: The Contractor shall provide nursingstaff who are competent, qualified per this performance work statement and adequately trained to perform assigned duties. Contractors staff shall be responsible for signing in and out when in attendance. Time sheets will be used by the Nurse Manager to confirm hours/day and services provided against the contractor's invoices.
  • Long Term Care Registered Nurse (RN) 8.0 FTE: The Long Term Care RN provides skilled nursing care to long term care patients with chronic illness, or disability and are not capable of caring for themselves for an extended period of time. The RN ensures patient centered, individualized care that promotes independence maximizes patient's quality of life, and ensures coordination of services to meet the patient's needs over a period of time.
  • Experienced and qualified Long Term Care Registered Nurses are required to provide additional staffing for the Inpatient Long Term Care Unit at the Finger Lakes VAMC. All registered nurses shall have at least two years recent experience in long term care. VA experience is preferred.
  • Educational Requirements: Minimum of Associate's Degree in Nursing (AND) or Bachelor's Degree in Nursing or a Master's Degree in Nursing. Contract nurses are preferred to have experience in long term care and accreditation through the American Nurses Credentialing Center (ANCC).
  • A current CPR/BLS certificate by the American Heart Association is required prior to starting work at FLHCS
  • A functional statement for contract CLC RNs can be found at D.1. The functional statement must be signed by each contract RN and returned to the VA when the candidate is presented for consideration. Failure to provide the functional statement may result in a delay in the candidate review process.

Supervisory Control: The Contract Registered Nurse is accountable to the Nurse Manager within the Community Living Center unit (CLC).Qualifications: Minimum of Associate Degree in Nursing (ADN) or bachelor's degree in Nursing (BSN) is preferred. Experience in Geriatrics and Extended Care and rehabilitation is preferred.Responsibilities:a. Utilizes the nursing process in managing, developing, implementing, and evaluating each resident's individual plan of care with the interdisciplinary team to attain specific clinical outcomes.b. Provides a full range of nursing care to residents with a variety of physical and/or behavioral problems.c. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others.d. Participates in quality improvement activities and the evaluation of caregiver issuese. Assesses learning needs of veterans, caregivers, and staff.f. Works collaboratively with all levels of health care professional and support staff to facilitate positive resident care outcomes.g. Effectively communicates with veterans, caregivers, visitors, and members of the interdisciplinary teamh. Guides and direct resident care delivery of support staffi. Administration & Monitoring of Medicationsj. BLS certification is required for this position and must be obtained prior to hirek. Compliance with various Isolation Precautions and Infection Control Guidelinesl. Skills in time management, priority setting, assessment and urgent intervention are essential for this unitm. Advanced Vascular Access Care (PICC/Ports) and Maintenancen. Trach careo. Maintains prevention & management of disruptive behavior skills, can be trained upon hirep. Patient and caregiver education and support.q. Participation in High Reliability Organization (HRO) & professional governance initiativesr. Promotes evidence-based best practices and nurse sensitive indicators (including but not limited to Falls Prevention, Infection Prevention, Medication error prevention, Safe Patient Handling, and Skin Safety.)QUALIFICATIONS:StaffLicense: Licensed personnel assigned by the Contractor to perform services covered by this contract shall be currently certified or licensed, as appropriate, in any State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on this contract shall be fully valid and without restriction. The qualifications of such personnel shall also be subject to review by the VA Nurse Executive and by the VA Facility Director. The Government specifically reserves the right to reject and of the Contractor's personnel and refuse them permission to provide services under this contract.Technical Proficiency: Contractors staff shall be technically proficient in the skills necessary to fulfill the government's requirements, including the ability to speak, understand, read and write English fluently. Contractor shall provide verifiable evidence of all educational and training experiences including any gaps in educational history for all Contractors staff and all Contractors staff shall be responsible for abiding by the Facility's Medical Staff By-Laws, rules, and regulations (referenced herein) that govern medical staff behavior.HOURS OF OPERATIONVA Business Hours: 24/7, including evenings, nights, weekends and Federal Holidays.Work Schedule: Contractor will be required to provide nursing staff to perform services at FLHCS. The schedule may change based on the need of the Government. All changes will be made through contract modifications. Staff will be offered to work either straight eight (8) hour shifts or twelve (12) hour shifts with one (1) eight (8) hour tour every two weeks. The following schedule options applies (note: if assigned to Bath CLC5 unit there is a special schedule set):
