Overview

Join to apply for the Screening Patient Navigator role at Westchester Medical Center Health Network.

The Patient Navigator helps to identify barriers that may prevent patients from receiving timely and high-quality healthcare treatment and works to find solutions to those barriers. The position helps navigate patients through the health care system, including providers' offices, hospitals, insurance and payment systems, patient-support organizations and other components of the health care system.

Responsibilities
  • Assists logistics coordinator with network wide escalations of discharge barriers.
  • Assists with expediting barriers to progression of care.
  • Assists in overcoming logistical barriers to scheduled diagnosis and treatment appointments.
  • Provides outreach to patients to help them understand the importance of preventive services, based on the age and condition(s).
  • Helps patients to understand recommended follow-up.
  • Contacts patients who are at risk for missing appointments.
  • Provides educational tools as indicated.
  • Facilitates access to available resources for obtaining insurance coverage and/or sliding fee benefits.
  • Assists to ensure that appropriate information is available in the patients medical records during scheduled appointments.
  • Facilitates linkages to follow-up services.
  • Coaches patients to become advocates for their own care.
  • Builds awareness of patient navigator services among the health care team by building professional relationships and by providing information about the service and maintaining communication to identify patients at risk for barriers to care.
  • Communicates with providers about unique patient needs (e.g., cultural considerations).
  • Other duties as assigned.
Qualifications/Requirements
  • Experience: Two years of experience in healthcare, required.
  • Knowledge about health care systems, the breadth and accessibility of community resources, and working knowledge of HIPAA regulations (preferred).
  • Education: High school diploma or equivalent required. Associates Degree, preferred.
  • Licenses / Certifications: N/A.
