Hospice Liaison – Community Liaison/Representative

Hospice Liaison Needed –Corpus Christi

As a Hospice Liaison (Community Liaison/Representative), you'll play a key role in increasing awareness and access to hospice services through education, outreach, and relationship-building across the local healthcare community.

This role is ideal for someone passionate about making a difference in the lives of patients and families while working collaboratively with an experienced and supportive team.

Salary and Benefits – Hospice Liaison

  • Performance-based compensation structure
  • $10,000 in annual salary per 1 average monthly admission (based on 3-month average length of stay)
  • Example:
    • Shorter LOS (1 month): ~25 admissions/month = $100,000/year
    • Longer LOS (6 months): ~6–7 admissions/month = $100,000/year
  • Full health, dental, and vision insurance
  • 401(k) with company contribution

Key Responsibilities – Community-Based Hospice Liaison

  • Monday to Friday schedule – 8:00 AM to 5:00 PM
  • No weekends or on-call
  • Build and maintain referral relationships with physicians, hospitals, ALFs, SNFs, other healthcare partners
  • Educate referral sources and community stakeholders about hospice services, eligibility criteria, and benefits
  • Conduct outreach through in-person visits, presentations, events, and networking opportunities
  • Coordinate closely with internal teams to support a seamless admissions process
  • Achieve or exceed monthly referral and admissions goals
  • Accurately document referral activity and outcomes using CRM tools
  • Stay informed on hospice industry trends, compliance regulations, and local market dynamics
  • Represent the organization professionally and compassionately in all community settings

Requirements

  • 1–3 years of experience in hospice, home health, or healthcare marketing
  • Strong knowledge of hospice philosophy and Medicare/Medicaid guidelines
  • Excellent communication, presentation, and relationship-building skills
  • Ability to work independently in the field and manage a referral territory
  • Valid driver's license and reliable transportation
  • Proven record of achieving referral or admissions goals
