SUMMARY:
  • The Post-Adoption Support Caseworker is responsible for assessing and providing voluntary services to Central Illinois families formed through adoption and subsidized guardianship. Services include in-home assessments, crisis and other counseling interventions, education, training, support groups, respite, advocacy and referrals to community services.
  • LOCATION: Based in Charleston/Matoon, IL with extensive field work required
  • WORK HOURS: Full-time, Flexible, 37.5 hours/week, must be available evenings to meet client needs
  • PAY: hourly, non-exempt, based on experience, education, and certifications. Master's Level applicants encouraged to apply with higher base rate.
Responsibilities:
  • Assessment: Conducts family and individual assessments of strengths and needs that are compliant with both ASAP procedures, DCFS, and COA.
  • Service/Treatment Planning: With the input of client families, plans services and treatments that will strengthen and support the families and focus child(ren). All Individualized Treatment Plans must be ASAP, DCFS, and COA compliant.
  • Mentoring, Advocating, Supporting, and Educating: In accordance with the service goals, provides mentoring, advocacy, support, and education that will protect children, preserve families and enhance the mental health of children and families alike.
  • Counseling Interventions: Provides appropriate counseling interventions that will support the focus child(ren) and their families to enhance their overall level of functioning.
  • Record keeping: Documentation must meet agency, DCFS and COA requirements for timeliness, completeness and accuracy.
  • Community Relations: Represents the agency in a positive light to active and potential clients, referral and funding sources, and the general public. Utilizes community contacts to advocate for clients and to promote child and family well-being.

  • Bachelor's Degree in social work or related field required.
  • Valid driver's license with adequate auto insurance. Must be comfortable driving long distances and entering clients' homes.
  • Must be available some evenings hours to meet needs of families
Salary Description $25.81/hour+ (equivalent to $50,336/yr minimum)
