Wellness Coach

Schedule: Tuesday–Saturday, 9:00 AM – 5:00 PM

Position Summary

Provide case management and supportive services to adults (ages 25–91) with mental health and substance use needs residing in permanent housing. Assist tenants in maintaining housing stability, accessing community resources, achieving personal goals, and promoting overall wellness.

Key Responsibilities

  • Manage a caseload of 15–18 tenants, developing and monitoring individualized support plans.
  • Assist tenants with problem-solving, crisis intervention, and daily living skills; provide hands-on support or referrals as needed.
  • Accompany tenants to medical, psychiatric, and public benefit appointments; advocate for tenants while encouraging self-advocacy.
  • Conduct home and hospital visits, apartment inspections, and assist with tenant moves, including packing and cleaning.
  • Collaborate with social service team to design and facilitate recreational, educational, and skill-building activities that foster community inclusion.
  • Serve as liaison to community-based services, including civic, social service, government, and business organizations.
  • Maintain relationships with therapeutic support services and ensure tenants attend required appointments, providing resources and referrals.
  • Support Facilities Management staff in ensuring maintenance of apartment standards as outlined by OMH.
  • Document all services in compliance with agency standards and participate in supervision, team meetings, case conferences, and trainings.
  • Adhere to on-call responsibilities for crisis intervention, emergencies, and urgent situations.

Qualifications

Education & Experience (Required)

  • Bachelor's degree
  • Proficient in email and Microsoft Office (Word, Excel, PowerPoint, Windows OS).

Preferred Skills & Experience

  • Two years' experience supporting individuals with mental illness, substance use issues, and/or a history of homelessness.
  • Experience in group work, tenant organization, case management, referral, and community outreach.
  • Background in supportive housing settings.
  • Bilingual in English/Spanish preferred.
