POSITION OVERVIEW
This is the perfect position for someone looking to enrich their career in human services by supporting the children in our residential care by coordinating services and life supports along with our clinical team. The Residential Case Manager will work out of our Albany, NY campus and will be responsible for supporting our clients in the Residential System while working collaboratively with the overseeing licensed clinicians.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Arrange and attend home visits with clients.
Prepare transition plans for after residential care ends.
Coordinate Independent Living Services for clients.
Act as the primary contact between the schools and Residential team.
Advise family in coordinated visits with clients.
Complete various assessments, including FASP and SPR.
Complete accurate case notes and timely input the notes into Connections database
Provide transportation for clients to service appointments.
Maintain contact with DSS and client's family.
Function as a contributing member of the interdisciplinary treatment team.
Attend dorm meetings and treatment review meetings.
EDUCATION and/or EXPERIENCE
Requires a bachelor's degree or commensurate level of education and related experience.
Experience with individuals in care systems is preferred but not necessary.
Valid Driver's license is required for this position.
This position requires 25 - 50% of community travel/client visits.
