Join our team today and immerse yourself in a rewarding career for years to come! As a Case Manager, you will work within our Housing Supports program located in Lawrenceville, Mercer County NJ. The Housing Supports program helps to find and maintain housing for insecure members to improve health outcomes and drive a greater connection of the housing and health care systems. Schedule: Full-time; Monday- Friday 9:00am-5:30pm; 30 minute unpaid lunch. Flexibility to meet needs of households and landlords including ability to respond to after-hours needs. Responsibilities:
  • Assists Program Coordinator in ensuring that program provides quality care which meets the standards of Oaks Integrated Care and its funding and regulatory sources
  • Collaborate with DCA and NJ FamilyCare to implement program expectations
  • Developing an individualized housing support plan with participants
  • Assisting with navigating the complexities of the housing application process through the progression of prospective tenant to tenant as well as assisting with the housing search
  • Identifying, coordinating, and securing resources to assist with housing costs and other expenses
  • Document all contacts in New Jersey's Homeless Management Information System (HMIS) or a HMIS-comparable system consistent with additional guidance from DMAHS
  • Assisting with the housing recertification processes, including lease renewals and housing subsidy renewals
  • Educating and training the participant on the role, rights and responsibilities of the tenant and landlord
  • Supporting the participant in development of independent living and tenancy skills, including: housekeeping; cleanliness; time management; financial literacy skills; budgeting; fraud prevention; establishing a bank account; connections to community services including grocery stores, transportation, schools, and jobs; as well as connecting the individual to social services
  • Participate in staff meeting and other program meetings/activities
  • Perform other duties as assigned
  • Use of personal vehicle may be required

Qualifications:
  • Bachelor's degree in social service field i.e. psychology, sociology, human services, criminal justice, social work.
  • Valid Driver's License required.
