Become the Transformative Case Manager You Aspire to Be
- Work collaboratively with an interdisciplinary team to develop treatment plans tailored to patient needs.
- Oversee the establishment of discharge plans and contingency protocols.
- Facilitate the execution of the patient discharge experience.
- Monitor and evaluate patient experience metrics: quality, timeliness, and appropriateness of care.
- Lead weekly team conferences to discuss treatment plan adjustments.
- Complete required case management documentation and addendums.
- Stay informed about regulations, company policies, and departmental operations.
- Review case management reports and strategize action plans based on Key Care Indicators.
- Understand commercial contract levels and payor requirements for compliance.
- Participate in Acute Care Transfer meetings to identify and mitigate trends.
- Meet with patients and families within 24 hours of admission to establish communication.
- Assess patient goals and complete necessary documentation within 48 hours of admission.
- Educate patients and families on rehabilitation processes and the role of the Case Manager.
- Schedule family conferences when necessary.
- Assist patients in procuring resources to prevent discharge delays.
- Ensure compliance with regulations for orthotics and prosthetics ordering.
- Make timely referrals and ensure documentation for post-discharge providers.
- Maintain accurate discharge and payor-related information.
- Engage in the utilization review process including data collection and trend analysis.
- Participate in the case management on-call schedule as needed.
Qualifications Required
- License or Certification:
- Must be able to perform independent assessments within the scope of practice.
- If licensure is required, an active license is necessary.
- Must meet eligibility for CCM® or ACM certification within two years of hire.
- CCM® or ACM certification required within two years of being appointed to this position.
Minimum Qualifications:
- Nursing candidates must have an Associate Degree in Nursing and an active RN license (BSN preferred).
- Other licensed or certified health care professionals must possess a minimum of a bachelor's degree (graduate degree preferred).
- A minimum of 2 years of rehabilitation experience is preferred.
