Your Responsibilities Include:
- Collaborate with an interdisciplinary team to develop person-centered treatment plans.
- Coordinate care from admission through discharge, ensuring a smooth transition.
- Monitor patient experiences and quality of care metrics.
- Lead weekly team conferences to adjust treatment plans based on patient needs.
- Maintain compliance with regulations and company policies.
- Engage with patients and families to educate and establish communication plans.
- Facilitate family meetings and coordinate resource procurement to prevent discharge delays.
- Participate in utilization review processes and case management on-call schedules.
Qualifications:
- Active license in your discipline (e.g., RN, SW, OT, PT, ST), with required qualifications to conduct assessments.
- CCM® or ACM certification is desirable or must be obtained within two years of hiring.
- For nursing candidates: A minimum of an Associate Degree in Nursing and RN licensure (BSN preferred).
- For other health professionals: A minimum of a bachelor's degree is required; graduate degrees are preferred.
- At least two years of experience in rehabilitation is preferred.
