Key Responsibilities:
- Conduct assessments, develop care plans, and track progress
- Coordinate medical, behavioral health, and social services
- Educate and support clients and families
- Advocate for high-quality, coordinated care
- Maintain accurate documentation
- Participate in team meetings and case conferences
Qualifications:
- Bachelor's in social work, nursing, psychology, or related field
- Experience with complex medical and behavioral health needs
- Strong communication and teamwork skills
- Proficiency in Microsoft Office & electronic health records
- Bilingual skills a plus
