Remote Patient Monitoring Home Telehealth Care Coordinator

Connected Care has one Registered Nurse vacancy for a Remote Patient Monitoring Home Telehealth Care Coordinator (RPM-HTCC). The Home Telehealth Care Coordinator (HTCC) is a Registered Nurse involved with the ongoing assessment,monitoring and case management of Veterans in their residential environment (or their environment of choice) and provides the appropriate information to providers and the healthcare system to enable just in time care.

Responsibilities HTCC serve as a case manager who utilize Home Telehealthtechnology in their practice of case management. Care Coordinators/Case Managers are in touch with Veterans in between their regular visits, advocating for the Veteran, ensuring the Veteran is receiving the care they need, and providing ongoing education and coaching as to improve the Veterans ability to manage their chronic conditions. The HTCC engages in activities to support not only the Veteran but informal caregivers and volunteers, making the home the preferred place of care for the Veteran and Caregiver in a pivotal position in the care delivery process.

Some of the duties of the position include but are not limited the following:

  • Triage and access all data received from Home Telehealth patients such as vital signs, reported symptoms, and question responses.
  • Contact patients with high-risk responses, significant changes in condition or changes in other specific data elements received.
  • Identify and intervene for potential exacerbations or complications to facilitate: Just in Time care in clinic, ER/Urgent care, or care in the community
  • Provider-directed interventions such as medication management
  • Protocol-based interventions.
  • Triage calls and concerns of patient/family, resolve those within scope of and route others to primary care staff or other services as indicated.
  • Provide opportunity for extension of length of time between routine primary care and/or HBPC encounters for stable patients.
  • Provide communication and data communication and data exchange with community-based providers when there is co-managed care.
  • Provide psychosocial support to patients.
  • Provide interdisciplinary consultation and interventions such as with HBPC and other non-institutional care programs and venues, mental health, social work, pharmacy, nutrition, etc.
  • Identify patients knowledge, health factors, skills, and behaviors that support self management and identify gaps therein.
  • Provide health care coaching, patient education, and motivational counseling.
  • Communicate with provider regarding progress to goals; patterns or trends of data, symptoms, or findings; concerning findings, need for provider assessment and/or interventions.
  • Facilitate and communicate treatment changes directed by the provider and provide follow up evaluation of the patient after the changes are implemented.

Preferred Experience: Independent worker, Strong initiative, Current Critical Care and/or current PACT experience. Team player, Multi-tasker, Recent process improvement ideas.

Requirements Conditions of Employment:

  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy

Qualifications Basic Requirements:

  • English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR
  • Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR
  • In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirements for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD).
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)
  • NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification.

Physical Requirements:

In accordance with VA Handbook 5019, Part II, Paragraph 3, II-1, 1(a), A pre-placement physical examination to determine the physical, cognitive and emotional fitness of candidates for appointment in VA is required, and shall be completed prior to appointment, for all full-time individuals appointed under 38 U.S.C., Chapter 73, for Title 38 Education.
