This is a remote position where you will work closely with community members enrolled in the Wisconsin IRIS Program and the TMG IRIS Consultant Agency. While your primary office will be your home, you will have regular, scheduled visits with IRIS participants in their homes and communities.

As an IRIS Consultant (IC), you will develop supportive relationships with your clients and assist them in navigating the Wisconsin IRIS program—a Medicaid long-term care option for older adults and individuals with disabilities. Together, you will identify their long-term care goals and craft innovative strategies to achieve those objectives.

Your role is essential in empowering individuals from diverse backgrounds to live the lives they choose. Our clients frequently express appreciation for the supportive nature and positive influence of our ICs. As an IC, you will connect participants with local resources and help them create personalized IRIS plans to reach their goals in areas such as employment, housing, health, safety, community involvement, transportation, and lasting relationships. KNOWLEDGE/SKILLS/ABILITIES

• Meet in person with IRIS participants at least four times a year, including one required annual visit at their home.
• Provide program orientation to new participants, covering their rights and responsibilities, legal document verification, and responsible use of public funds.
• Explore all aspects of the participant's life, including personal goals, significant relationships, community connections, employment interests, and back-up support systems.
• Assist participants in identifying and continually achieving personal outcomes while adhering to their IRIS budget and program regulations set by the Department of Health Services (DHS).
• Document all orientation and planning activities within the IRIS data system (WISITs) within 48 business hours of your visit.
• Research community resources that align with individual outcomes and promptly share that information with participants.
• Document any progress or changes needed within the participant's plan and data system upon modification requests.
• Handle Budget Amendment or One-Time Expense paperwork as necessary based on individual participant requirements.
• Educate participants on interpreting their monthly budget reports to ensure compliance and spend within their budget while serving as a liaison between the Fiscal Employer Agency and the IRIS Consultant Agency.
• Build engaged and trusting relationships with participants, effectively communicating any program changes and compliance requirements.
• Uphold confidentiality and HIPAA compliance at all times.
• Collaborate with other IRIS Consultant Agency staff for successful participant plan implementation.
• Attend monthly team meetings and have weekly check-ins with supervisors, along with fulfilling other assigned duties.

JOB QUALIFICATIONS

Required Education

Bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field (or four years of equivalent experience if no degree).

Required Experience

• 1+ year of direct experience in delivering social services to individuals with intellectual or physical disabilities and older adults.
• Able to work independently with minimal supervision; self-motivated.
• Knowledge of Long-Term Care programs and familiarity with self-determination principles.
• Strong problem-solving and critical thinking skills along with basic math skills.
• Excellent time management and prioritization abilities to handle multiple projects and adapt to changes.
• Skilled in building and maintaining professional relationships and addressing situations without personal bias.
• Adaptable to various work environments and able to engage with a diverse population and personal circumstances.
• Resourceful and knowledgeable about community resources; proactive and detail-oriented.
• Proficient in using technology, including Outlook, Skype, Teams, PowerPoint, Excel, Word, and online databases.

Required License, Certification, Association

Must hold a valid driver's license with a good driving record and have reliable transportation for driving within applicable states.

Pay Range: $18.04 - $35.17/HOURLY. Actual compensation may vary based on geographic location, work experience, education, and skill level.
