This is a remote position where you will work closely with community members enrolled in the Wisconsin IRIS Program and the TMG IRIS Consultant Agency. While your primary office will be your home, you will have regular, scheduled visits with IRIS participants in their homes and communities.
As an IRIS Consultant (IC), you will develop supportive relationships with your clients and assist them in navigating the Wisconsin IRIS program—a Medicaid long-term care option for older adults and individuals with disabilities. Together, you will identify their long-term care goals and craft innovative strategies to achieve those objectives.
Your role is essential in empowering individuals from diverse backgrounds to live the lives they choose. Our clients frequently express appreciation for the supportive nature and positive influence of our ICs. We seek compassionate, resourceful individuals who value high-quality work and are eager to collaborate closely with IRIS participants.
As an IC, you will connect participants with local resources and help them create personalized IRIS plans to reach their goals in areas such as employment, housing, health, safety, community involvement, transportation, and lasting relationships. Although your tasks may follow a certain routine, each day will present unique challenges and opportunities!

KNOWLEDGE/SKILLS/ABILITIES
JOB QUALIFICATIONS
Required Education
Bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field (or four years of equivalent experience if no degree).
Required Experience
Required License, Certification, Association
Must hold a valid driver's license with a good driving record and have reliable transportation for driving within applicable states.

Pay Range: $18.04 - $35.17/HOURLY. Actual compensation may vary based on geographic location, work experience, education, and skill level.
