Job Description :
Description 30 hours per week / benefited position. Monday - Friday Support Adult Day Health program participants as Activities Coordinator in Gardner. Provides participant care consistent with the plan of care, while encouraging self-help measures and independence. Coordinate and lead a broad variety of regularly scheduled, purposeful activities including recreational, spiritual, educational, social, crafts and exercise program. DUTIES AND RESPONSIBILITIES: 1. Develops an activity program in conjunction with an occupational therapist. 2. Schedules a monthly activity program including individual or group activities. 3. Prepare, distribute and post a monthly activity calendar. 4. Supervises and conducts activities and/or coordinates day-to-day implementation of daily activities. 5. Observes all activities to ensure the protection and safety of all participants. 6. Oversees the work of activity volunteers. 7. Utilizes community resources to enhance programming and to promote facility-community relationships. 8. Documents in the client record upon admission, at least quarterly (monthly if client is a social client). Notes should reflect the client's plan of care and client's response to activities. 9. Provide assistance with client's personal needs. 10. Participate in regularly scheduled activity coordinator meetings and in-service education programs to maintain job skills and knowledge. Requirements
  • Certification in activity preferred.
  • Two (2) years' experience in caring for the elderly.
  • Ability to read, write, speak and understand English clearly including basic math skills.
  • Skilled in developing and implementing a therapeutic activity program.
  • Good communication skills
  • Evidence of maturity and ability to deal effectively with job demands and stress.
  • Ability to project a positive image of the CCVNA and maintain a customer-focused approach to all work.
  • Ability to work independently.
  • REQUIREMENTS: 1. Must have flexibility, personal integrity, and the ability to work effectively with clients and CCVNA staff. 2. Must be able to relate to the ill, disabled, emotionally upset and individuals with special needs. 3. CPR and First Aid certification when provided by the agency.
