Position Summary
  • This position will serve to support the discharge planning process across ECU Health by coordinating post discharge services and assisting with oversight by the Nurse Care Managers (NCM) and/or the Social Work Care Managers (SWCM) within the Case Management.
  • This position may assist in collection of patient information, discharge care needs, regulatory interventions as well as work to facilitate a transition plan that promotes a safe and effective discharge and utilization of services.
  • This position will perform other activities related to the discharge planning as requested.

  • Monday to Friday 8:00am - 5:00pm
  • Weekend on-call coverage
  • Holiday rotation
  • On-Site
