Description:

POSITION PURPOSE:

As a Case Manager within our Community Treatment Team, you will be responsible for providing comprehensive case management services to individuals with mental health and behavioral health needs within the community as well as a combination of office-based work. You will work closely with clients, their families, healthcare providers, and other community resources to ensure they receive appropriate and timely support and services.

WHAT YOU WILL DO:

Assessment and Planning: Conduct thorough assessments of clients' needs and develop individualized care plans in collaboration with the treatment team. Coordination of Services: Coordinate and facilitate access to a range of community-based services including mental health treatment, housing assistance, vocational support, substance abuse treatment, and other essential services. Client Advocacy: Advocate on behalf of clients to ensure their rights are upheld and they receive appropriate care and support. Monitoring and Follow-up: Regularly monitor clients' progress, conduct follow-up assessments, and adjust care plans as needed to ensure clients' needs are met effectively. Crisis Intervention: Provide crisis intervention services as needed and collaborate with emergency services and crisis teams to ensure clients' safety. Documentation and Reporting: Maintain accurate and up-to-date documentation of client interactions, service referrals, and progress notes in accordance with agency policies and regulatory requirements. Team Collaboration: Work collaboratively with other members of the Community Treatment Team, including psychiatrists, therapists, nurses, and peer support specialists, to provide comprehensive care to clients. Community Outreach: Engage in community outreach activities to raise awareness about mental health issues, reduce stigma, and promote access to services.

MINIMUM QUALIFICATIONS:

  • Bachelors in Human Service such as; sociology, social work, psychology, criminal justice, gerontology, counseling, education or nursing and (3) years of experience in a related field OR Masters in Human Services such as; sociology, social work, psychology, criminal justice, gerontology, counseling, education or nursing and1 year of experience in a related field.

  • Act 33/34/73 clearances.

  • Pre-employment Drug Screen

  • Valid driver's license

  • Ability to drive an organization vehicle

POSITION HIGHIGHTS/BENEFITS

WORK SCHEDULE: This is an Exempt (salary) position. Monday through Friday daylight with flexibility to meet consumers' needs. Nontraditional hours (evenings, weekends, and holidays) may be required. Will provide on call services on a rotating basis ($50.00 flat rate daily)
