Job Description :

Lutheran Social Services is eager to welcome a dedicated Service Coordinator to our Comprehensive Community Services team! This role is essential in our team-based wraparound program designed for individuals with mental health needs. The Service Coordinator will provide community-based services to clients experiencing significant challenges within their homes, schools, and communities. This full-time position requires 40 hours a week and includes a range of employee benefits!

Under the guidance of a Licensed Therapist or qualified Licensed Psychologist, the Service Coordinator will conduct clinical assessments, diagnostics, and treatment for individuals facing mental health, substance use, or co-occurring disorders.

Key Responsibilities:

  • Conduct comprehensive assessments, identifying strengths, challenges, and recovery objectives.
  • Develop detailed service plans that set measurable goals and regular reviews to track progress.
  • Facilitate smooth service delivery to ensure clients receive timely assistance in line with their service plans.

Essential Duties:

  • Deliver assigned services to clients based on individualized service plans.
  • Provide psychotherapy services to individuals, couples, and families in various settings including schools, offices, and homes.
  • Perform client assessments using interviews and standardized tools.
  • Collaborate with clients and other professionals to create treatment plans.
  • Offer therapeutic services adhering to best practice standards.
  • Maintain accurate documentation within required timelines.
  • Participate in client and recovery team meetings.
  • Engage in ongoing professional development and supervision.
  • Adapt working hours to fit client needs, including evenings or weekends as necessary.
  • Meet established productivity expectations for your role.
  • Other duties and special projects as required.

Qualifications:

  • A Master's degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or a related field.
  • Relevant practicum experience related to the services provided.
  • Eligibility for a training license as prescribed by the WI Department of Safety and Professional Services.

Licensure and Skills:

  • Must possess or be eligible for appropriate state licensure.
  • Valid driver's license and reliable transportation are required.
  • Strong written and verbal communication skills.
  • Proficient in using computers and electronic health records for documentation.

Work Environment:

This position requires travel to client homes, and the incumbent should be prepared for varied environmental conditions. The role may involve exposure to unsanitary conditions or allergens, and the ability to respond competently to crises is essential.
