RiverSpring Living provides a full range of care solutions including nursing home care, managed long term care, assisted living programs, senior housing, rehabilitation services and specialized services such as elder abuse prevention and memory care.RiverSpring Certified Home Health Agency provides skilled home services, including nursing, physical therapy, occupational therapy, speech therapy, and social work as well as home health aide services.Key Responsibilities:The LPN Intake Coordinator acts as a professional resource to the facility that manages/facilitates the safe and timely transfer/referral process of patients from inpatient or community settings to the care of RiverSpring CHHA. The LPN Intake Coordinator will oversee the screening, evaluation, and admission planning for our CHHA Program.This role involves managing the entire process from inquiry to admission acceptance and coordinating the scheduling of Nurse Care Managers and vendors for timely evaluations. The LPN Intake Coordinator will work closely with the Assistant Director of Administrative Operations to establish new referral sources. Qualifications:Graduate of an approved school of nursing whose program is accredited by the National League of Nursing. Current, unencumbered NYS LPN LicenseMinimum of 2 years of experience in hospital or home care agency reviewing patients for home care safety and assisting with discharge planning.Ability to prioritize tasks; Able to multitask efficiently and effectively.Excellent verbal and written skills with the ability to communicate information concerning a patient's condition clearly.Performs on-call duties as assigned.Performs other duties as assigned.Knowledge of nursing and medical practices, procedures, regulations and guidelines that pertain to homecare.Patient Review Instrument (PRI) assessment experience preferredValid driver's license and proficiency in MS Office requiredCompensation:Annual salary: $80,000 - $85,000Benefits include health insurance, retirement plans, paid time off, and professional development opportunitiesIf you're ready to advance your career and make a significant impact in the field of home health care, we would love to hear from you!Equal Opportunity Employer (EOE)
