Distributes appropriate information to ancillary departments. Participates in performance improvement and CQI activities.Position Responsibilities:Clinical / Technical Skills (40% of performance review)Coordinates transportation, and communicates details to patients and staffResponsible for following up on vehicle maintenance needsMaintains transportation/maintenance logsFollow up with patients when absent from group, and provide reminder calls as necessaryFollow up on all inpatient referrals; stepdown admissionsProvides customer service and follow up with inpatient therapist on referralsKeeping therapist informed of referral and patient status. Visit with potential outpatient referrals in IntakeAssists in maintaining patient charts, to include admission and insurance data.Answer phones in a professional manner, and answers questions as appropriateEscort patients to and from lunchCollect google reviews form patientsAssist patient when completing Basis 32Check in patients into the Outpatient department, and enters information in to EMRCollect co pays from patientsFax pre-certs to insurance companiesAssist in transportation duties, as necessaryResponsible for knowing hospital policies and being familiar with hospital services that are available.Refers patient to the Business Office when financial arrangements need to be made.Demonstrates an ability to be flexible, organized and function well in stressful situations.Ensures that documentation meets current standards and policies.Perform other duties as assigned.Safety (15% of performance review)Strives to create a safe, healing environment for patients and family membersFollows all safety rules while on the job.Reports near misses, as well as errors and accidents promptly.Corrects minor safety hazards.Communicates with peers and management regarding any hazards identified in the workplace.Attends all required safety programs and understands responsibilities related to general, department, and job specific safety.Participates in quality projects, as assigned, and supports quality initiatives.Supports and maintains a culture of safety and quality.Teamwork (15% of performance review)Works well with others in a spirit of teamwork and cooperation.
